Premium Structure: - Unisex rates
- Issue-Age rating (age of application date, age bands)
- Occupation class determined by level of risk (2A, A, B)
- Monthly Benefit Amount
| Member Selects: - Elimination Period- The # of days the insured must be totally disabled before any benefits are paid
- 14 days or 30 days
- Benefit Period- The amount of time the benefit payments will be sent directly to the insured (begins upon completion of elimination period)
- 12 months or 24 months

Employment: - Are you a full-time employee, working at least 30 hours per week?
Underwriting: (Standard questions, additional questions may apply) - 1a Do you have any other individual Disability Income Insurance in force or applied for, including salary continuation or sick leave?
- 2 Are you a full-time employee, working at least 30 hours per week?
- 3 Have you missed more than 5 consecutive workdays for any reason other than vacation or pregnancy in the last 6 months?
- 4 In the past 6 months have you been confined to a hospital as an inpatient or been a patient in a hospital emergency room?
- 5 In the past 2 years, been diagnosed or treated for disease or disorder of the back, neck, knee, spine or discs?
- 6 Ever been diagnosed as having or been treated for Acquired Immune Deficiency Syndrome (AIDS)?
- 7 In the past 2 years, been counseled, treated or convicted for use of alcohol and/or drugs?

Exclusions: (States may vary) - Some Occupations excluded
- Intentionally self-inflicted injury;
- Cosmetic surgery (except for reconstructive surgery needed as the result of an Injury) or other elective procedures not medically necessary;
- Mental illness without demonstrable organic cause;
- Normal pregnancy or childbirth;
- Complications of Pregnancy within 10 months of the policy effective date (Select States)
- No benefits will be payable under the Policy for the first 12 months from the Effective Date if the insured become totally disabled because of a pre-existing condition. At the end of such 12 month period, Combined Insurance will pay Total Disability benefits described under the policy.
